Please visit System Documents Site and follow the steps mentioned below to download the application form, fill in the form with required information, and send it to "j-gate@jpx.co.jp" by email.
[Click "J-Gate" at the bottom of system documents]
→ [Enter login ID and password on the login screen]
→ [Click the login button]
→ Click [Current J-Gate] on the left side of the screen
→ Click [Application/Usage Fee Related]
→ Click the 【2017/02/27updated】 J-GATE Login Password Reset Unlock Application Form to download the file.
