1: Application Type
Tick "Add" in the row of applying system and virtual server for new application.
Tick "Delete" in the row of applying system and virtual server for removal application.
 
2: Application Qty
Enter the quantity to Add in the row of applying system and virtual server for new application.
Enter the quantity to Delete in the row of applying system and virtual server for removal application.
* Not required for FLEX.
 
3: Desired Start Date
Select the start/cancel date from the calendar.
You can also enter the date directly in yyyy/mm/dd format.
For removal, enter the last date of use.
