1: Create the application form
Create the line application based on the contents of quote.
In the case of new/change, fill in the blank(s) on the application page if necessary.
In the case of cancellation, confirm the content and submit the application.
*Carrier may contact the Issuer (which is showed at the top of the application screen).
If the point of contact is not the Issuer, please input the contact information of the person in charge (name, phone number, e-mail address) in the "Special Notes" section at the bottom of the application screen.
If you can't put the information on "Special Note" due to exceeding the number of characters, etc., please send e-mail to JPX Service Desk (application procedure) <servicedesk@jpx.co.jp> with Application ID and contacts (name, phone, e-mail).
 
2: Application date
The date when the quote request was submitted.
 
3: Quote Valid until
Period of validity for the quote.
If an application is not submitted by the date, the quote request has to be submitted again.
 
4: Application ID
The identification code for applications.
The ID is displayed on the upper-left corner of the application form screen.
 
5: Form Name
The name of the application form or the request form.
 
 
6: Created by
The date when the quote request was submitted.
 
7: Status
Please see Status Guidance.
 
8: Project ID
Arbitrary code set by user as an identifier to bind multiple applications.
 
9: Carrier
The carrier designated in the quote condition.
 
10: Bandwidth
The bandwidth designated in the quote condition.
 
11: Line Installation Address
The line installation address designated in the quote condition.
 
12: Line Installation Building/Floor
The line installation building and floor designated in the quote condition.
