What's the procedure to apply for the connection to user premises through arrowface?

[Line Contracting User]
A user installing lines under user's own name and directly connecting to each SP
 
[Shared-System Service Provider] 
An ISV, etc. installing lines under its own name and providing Shared-System Service Users with the connection environment to each SP
 
[Shared-System Service User] 
A user connecting to each SP using the connection environment offered by Shared-System Service Provider
Line Contracting User
